The Evolution of Blood Transfusion: From Early Experiments to Life-Saving Techniques

Several decades after William Harvey demonstrated the circulation of blood, early attempts at blood transfusion began. In 1665, Richard Lower successfully kept dogs alive by transfusing blood from other dogs. Two years later, Jean-Baptiste Denys conducted the first documented blood transfusion in a human subject. While some single transfusions proved successful, recipients often experienced severe complications after their second or third transfusion. By the end of the seventeenth century, blood transfusions were banned and faded into obscurity until 1818, when English obstetrician James Blundell performed the first successful human blood transfusion to treat postpartum bleeding. Between 1825 and 1830, he carried out ten transfusions, five of which yielded positive results. Blundell not only achieved medical success but also gained financial reward, earning an equivalent of approximately $50 million today from his invention of blood transfusion instruments.

The discovery of blood types by Austrian-born American immunologist Karl Landsteiner revolutionized blood transfusions, making them a routine medical procedure. In 1901, while at Vienna’s Institute of Pathology, Landsteiner found that the blood of some individuals could agglutinate (clump) with the blood of others, leading to potentially fatal reactions due to an immunological (antigen-antibody) response. He identified three human blood groups: A, B, and C (which was later renamed O), and later discovered a fourth group, AB.

Landsteiner's blood typing laid the groundwork for the first successful transfusions of compatible blood, conducted in 1907 at Mt. Sinai Hospital in New York, and later on a large scale during World War I. By 1927, ABO blood types were being utilized in paternity suits to determine the biological parents of children. For his groundbreaking discovery of human blood groups and the ABO blood typing system, Landsteiner was awarded the Nobel Prize in 1930. In 1940, while working at the Rockefeller Institute (now Rockefeller University), he identified the Rh factor in Rhesus monkeys, which was linked to hemolytic disease of the newborn—a potentially life-threatening condition occurring when a mother and her fetus have incompatible blood types.


In 1901, Landsteiner identified the four human blood types, and four decades later, he discovered the Rh factor. In 1968, the introduction of the drug RhoGAM effectively suppressed the normal production of antibodies in Rh– mothers against their Rh+ fetus's red blood cells, successfully preventing hemolytic disease of the newborn.

Revolutionizing Anatomy: Vesalius, Galen, and the Renaissance of Medical Education

Anatomy knowledge serves as a cornerstone in medical education, considered indispensable for the diagnosis and treatment of diseases. It also holds significance for artists, particularly sculptors and painters. During the 16th century, when the Flemish anatomist Andreas Vesalius assumed a professorship at the University of Padua, a renowned hub for medical education, the teaching of anatomy relied heavily on the ancient texts of Galen, composed nearly fifteen centuries earlier. These traditional teachings involved readings from Galen's works, followed by a dissection conducted by a barber-surgeon under the lecturer's guidance.

Vesalius, however, broke away from this convention. He personally conducted dissections on cadavers, with his students gathered around the dissection table. What Vesalius observed during these dissections often contradicted Galen's long-established descriptions.

Galen, a preeminent scholar of antiquity and the physician to Pergamon's gladiators, had the opportunity to examine numerous human subjects. Nonetheless, due to the prohibition of human dissections in Ancient Rome, Galen created his anatomical illustrations using Barbary apes, claiming their similarity to humans.

In 1543, at the age of 28, Vesalius published the first edition of "De humani corporis fabrica." This groundbreaking work presented a comprehensive depiction of the human body, featuring the first detailed illustrations of internal organs. The book, featuring two hundred woodcuts, prioritized both didactic accuracy and aesthetic appeal. Vesalius, a perfectionist, insisted on the artwork being aesthetically pleasing. The collaborative effort between the dissector and the illustrator resulted in woodcuts historically attributed to Jan Stephen van Calcar, a student of the Italian Renaissance painter Titian.

Vesalius envisioned his book's audience not only as physicians and anatomists but also as artists. Despite initial resistance to his challenge to Galen's teachings, Vesalius's book catapulted him to fame and fortune. Today, it stands as one of the most renowned works in the fields of medicine and science. Of the approximately 500 copies originally printed, 130 still exist. Tragically, in 1564, Vesalius met his demise in a shipwreck in the Ionian Sea near the Greek Isle of Zakynthos (Zante) while returning from a pilgrimage to Jerusalem.

The Enduring Legacy of Humorism in Medicine: A Journey Through Millennia

For over two millennia, both Eastern and Western medical practitioners adhered to the idea that a harmonious equilibrium of four bodily humors, or "fluids," played a significant role in shaping our physical and mental health. The origins of this concept, known as the four humors, can be traced back to ancient Egypt and Mesopotamia. It was Hippocrates, during the fourth century BCE, who formalized and integrated these ideas into medical practice, giving rise to the system of humorism. According to this theory, good health depended on maintaining a balance among four essential fluids: blood, phlegm, black bile, and yellow bile. Any excess or deficiency of these humors was believed to lead to illness.


During ancient times, melancholy was thought to result from an excess of the "black bile" humor, and people believed that the planets had the power to restore the balance of these bodily humors. This particular stained glass artwork, created in the southern Netherlands around 1530, bears the title "The Planet Saturn Expelling a Monk with a Pig's Head, or Time Banishing Melancholy."

This revolutionary concept replaced the earlier prevailing belief that diseases were caused by malevolent supernatural spirits. Over time, humorism gained widespread acceptance and became the foundation of medical treatment in the Greco-Roman world, championed by figures like Galen. Galen further postulated that imbalances in the humors were responsible for distinct temperamental traits, categorizing individuals as sanguine (associated with blood), phlegmatic (related to phlegm), melancholic (tied to black bile), or choleric (linked to yellow bile). Humorism gradually spread across various regions, adapting to local influences, reaching as far as the Muslim world, China, India, and Western Europe. In "The Canon of Medicine" (1025), the renowned Muslim physician Avicenna expanded upon the connection between humor imbalances, changes in temperament, and the onset of diseases, proposing an association with the vital organs—the brain and heart.

During the Elizabethan era, efforts were made to maintain humor balance by making dietary adjustments, engaging in specific exercises, choosing appropriate clothing, and even modifying bathing practices. Interestingly, bathing was considered more detrimental to men than women during this period.

In Western Europe, a medical practice known as "heroic medicine" emerged. It aimed to restore humor balance through extreme measures such as purging, inducing vomiting, and bloodletting. Tragically, George Washington's death in 1799 is often attributed, at least in part, to the excessive bloodletting he endured, losing approximately half of his total blood volume. Less aggressive treatments involved applying heat, cold, moisture, or dryness to restore equilibrium. Humorism remained the dominant paradigm for understanding well-being and medical practice until the nineteenth century when modern medical theories, rooted in advances in cellular pathology and the discovery of biological and bacterial causes of disease, gained widespread acceptance.
